| Following hospitalization for surgery, illness or injury, some people are not quite ready to return home and function independently. To transition patients from hospital to home, Saint Therese offers a designated 49-bed unit and outstanding specialized support, the ability to handle complex nursing needs, and rehabilitation services called Transitional Care.
This higher level of care involves an entire team of care professionals from rehabilitation therapy, nursing, social services, therapeutic recreation, pastoral care and nutritional services. The team works together with residents and families to design an individualized program.
